Request for a consultation (general request form), which contains: 1.1 text written in Lithuanian language or have a translation into Lithuanian language, which authenticity must be certifiedin the prescribed order by the Law on the Notary of the Republic of Lithuania
1.3.recipient of service: name, surname, place of residence (if recipient of service is citizen) or company name, company’sregistration code, address (if recipient of service is business), contact details and signature of recipient of service. 2. Electronically written request must be signed by a qualified electronic signature or formed by electronic means, which makes it possible to ensure the integrity and irreplaceability of the text 3. Request sent by e-mail must be sent to the official e-mail address, specified on the home page of website of the National Public Health Center under the Ministry of Health 4. When the representative of a person applies to National Public Health Center under the Ministry of Health on behalf of the person represented, there must be indicated name, surname, place of residence (by which a person wants to get an answer) as well as the name and surname of represented person (if applied on behalf of individual) or name (if applied on behalf of legal entity). Representation confirming document (or the copy) must be submitted. 5.
