Should the name of the activity carried out be indicated in the permit-hygiene passport (PHP)?

Yes. The specific activity must be indicated according to the list of types of economic/commercial activities, which require a permit-hygiene passport, specified in Article 21(4) of the Law on Public Health Care of the Republic of Lithuania and not according to the types of activities indicated in the classification of types of economic activities.
