1) Activities of outpatient institutions of personal health care (according to the list of licensed outpatient personal health care services approved by the Minister of Health*)
2) Activities of hospitals (according to the list of licensed inpatient personal health care services approved by the Minister of Health*)
3) Other personal health care activities approved by the Minister of Health*

4) Education activities: pre-school education activities, education activities of general education schools, education activities of children's non-formal education schools, education activities of vocational schools, education activities of higher education institutions
5) Activities of residential adult care and nursing institutions, activities of residential child and youth care and nursing institutions
6) Accommodation service activities (activities of hotels, motels, guesthouses, resort sanatoriums, resort rehabilitation centres, camping sites, tourist centres, rest houses, youth hostels)
7) Beauty service activities (hair care services, face care services, body care services, nail care services, tattoo services, permanent makeup services, piercing services)
8) Service activities of solariums
9) Service activities of sports clubs
10) Service activities of bathhouses, saunas, swimming pools
11) Service activities of laundries
12) Manufacture of cosmetic products
13) Activities relating to the provision of funeral services (storage of human corpses, their preparation for viewing, and laying out), cremation activities, embalming activities
14) Activities of stationary sources of noise in noise abatement zones
15) Activities of health services that use complementary alternative health care products, animals and other living organisms and which are not licensed under the Law on Complementary and Alternative Health Care.

Not necessarily. All procedures and formalities related to submission of the documents required for obtaining a permit-hygiene passport and decisions related to granting, correcting and obtaining a permit-hygiene passport, or a duplicate, can be completed:
1. Upon arrival at the appropriate NVSC district department, and upon presentation of an identity document (identity card or passport) and filling out the application.
2. By sending an application and a copy of the document confirming the identity of the person (identity card or passport), approved in accordance with the procedure established by legal acts, by mail or by courier to NVSC to the following address: 153 Kalvarijų St., Vilnius, or to the appropriate NVSC district department.
3. Through National information system for delivery of electronic messages and electronic documents using post infrastructure (eDelivery), where the identity of the person will be confirmed.
4. By sending an application form to NVSC by e-mail by mail [email protected]. When submitting an application form by e-mail, it must be signed using a qualified electronic signature that meets the requirements set out in Regulation (EU) No 910/2014, or in a way that allows the person submitting an application to be identified or to verify the authenticity of the application.
5. Through administrative and public e-services portal E-Government Gateway epaslaugos.lt, where the identity of the person will be confirmed (suspension of permits-hygiene passports, revocation of permits-hygiene passports, revocation of suspension of permits-hygiene passports when specifying the number of permit-hygiene passport, date of issuance, suspension, revocation or revocation of suspension, correction of permit-hygiene passport or issuance of duplicate of permit-hygiene passport).

2. Natural persons should submit a copy of the document proving their identity.
3. The specific activity must be indicated according to the list of types of economic/commercial activities, which require a permit-hygiene passport, specified in Article 21(4) of the Law on Public Health Care of the Republic of Lithuania and not according to the types of activities indicated in the classification of types of economic activities.
No. Environmental noise measurements must be taken in the premises defined in Lithuanian Hygiene Standard, which regulates noise emission limits, for certain activities specified in Article 21(4.2, 4.4-6) of the Law on Public Health of the Republic of Lithuania (activities of hospitals, educational activities, stationary social service institutions, accommodation services).

A permit-hygiene passport (PHP) is corrected by issuing a new permit-hygiene passport if the data specified in the permit-hygiene passport (PHP) changes, i.e. name of the legal entity or branch, address of the registered office, code (a new code is provided), personal data, correspondence address or other data (details) not related to the conditions of the economic/ commercial activity being carried out, after reorganisation of a legal entity, if the location of economic/ commercial activity does not change, as well as to correct errors made when issuing a permit-hygiene passport (PHP).
A natural person, a legal entities or a branch that holds a permit-hygiene passport, upon completion of reconstruction or repair work of the premises where economic/commercial activities are carried out which require the procedure of acceptance as fit for use, must immediately apply to the authority issuing permits-hygiene passports for the issuance of a new permit-hygiene passport (PHP).

1. A natural person, a legal entities or a branch that holds a permit-hygiene passport (PHP) must comply with the requirements of legal acts regulating public health safety, applicable to their economic/ commercial activities, and carry out economic/commercial activities under the conditions declared in the application for a permit-hygiene passport ( PHP), except for the cases when the changed conditions of economic/commercial activity have been reported to the institution that issued the permit-hygiene passport (PHP) in accordance with the procedure established in Clause 48.2 of the Rules, and the change in the conditions of economic/commercial activity does not violate the requirements of public health safety legislation.
2. A natural person, a legal entities or a branch that holds a permit-hygiene passport (PHP) must inform the institution that issued the permit-hygiene passport about:
Changed data recorded in the permit-hygiene passport (PHP) (legal entity and branch – name, registered office address, changed code (given new code); natural person – personal data, correspondence address) no later than within 10 working days from the change of these data.
Changed conditions for performance of economic and commercial activities no later than within 3 working days from the change of such conditions.
3. PHP holder must not authorize or transfer the right to engage in the activities specified in the PHP to other persons under the contract.
4. The original PHP or its copy must be presented in a place visible to users (the published PHP original or its copy should not show the personal identification number and address of the natural person).
5. In case of temporary cessation of the activity for which the PHP was issued, the PHP holder must submit an application to the NVSC for suspension of PHP.
6. Upon termination of the activity for which the PHP was issued, the PHP holder must submit an application to the NVSC for revocation of the PHP. Such an application must also be submitted when a new PHP is issued to a person who had already obtained an PHP after starting to provide new services in existing premises or starting to operate in existing premises where no activity was previously carried out (in this case, an application for revocation of the old PHP is submitted).
