Description of process for obtaining a service

1. Recipient of service submits request for consultation

2. Provider of service prepares an answer;

3. Provider of service sends answer to recipient of service

List of documents to be submitted to obtain a permit

1. Request for a consultation (general request form), which contains:

1.1 text written in Lithuanian language or have a translation into Lithuanian language, which authenticity must be certified in the prescribed order by the Law on the Notary of the Republic of Lithuania

    1. text is readable
1.3. recipient of service: name, surname, place of residence (if recipient of service is citizen) or company name, company’s registration code, address (if recipient of service is business), contact details and signature of recipient of service.
2. Electronically written request must be signed by a qualified electronic signature or formed by electronic means, which makes it possible to ensure the integrity and irreplaceability of the text
3. Request sent by e-mail must be sent to the official e-mail address, specified on the home page of website of the National Public Health Center under the Ministry of Health 
4. When the representative of a person applies to  National Public Health Center under the Ministry of Health  on behalf of the person represented, there must be indicated name, surname, place of residence (by which a person wants to get an answer) as well as the name and surname of represented person (if applied on behalf of individual) or name (if applied on behalf of legal entity). Representation confirming document (or the copy) must be submitted.
5. Written request can also be submitted by the recipient of service (or its representative) at the reception of National Public Health Center under the Ministry of Health

Information concerning available remedies

Recipient of service has the right to submit a complaint to the Administrative Disputes Commission in accordance with the procedure established by the Law on Administrative Dispute Commissions of the Republic of Lithuania and to the Administrative Court in accordance with the Law on Administrative Proceedings of the Republic of Lithuania.
