  2. Provider of service reviews submitted documents
  3. A decision on the possibilities of the proposed economic activity is given to the recipient of service (or a reasoned demand to correct or supplement a report)

  2. Public health impact assessment report
  3. Information validated by ward (eldership) on public expositions of report, including the address, beginning and end of the public exposition
  4. All documents related to the notification of citizens (publications in the media, copies of documents confirming the sending of registered letters, information on public expositions of report, protocol of meeting of reports introduction to citizens, copies of suggestions from citizens, responses to suggestions from citizens, document of evaluation of suggestions from citizens
