Beneficiary of of the fee is State Tax Inspectorate under the Ministry of Finance of the Republic of Lithuania, identification code 188659752. The payment shall be made to State Tax Inspectorate under the Ministry of Finance of the Republic of Lithuania indicating:

  • the name of the beneficiary
  • account number of budget collection account
  • payer’s identification code
  • payers name
  • payment code
  • payable amount

If taxes, fees and contributions are paid for other persons (businesses or persons), the extended payment order form, including the identification code, name and address of the originator of the payment shall be used

7.

Description of process for obtaining a service

  1. Applicant submits application and required documents
  2. Institution issuing permit-hygiene passport assesses submitted documents (if the application is not properly completed - requests for corrections, if some documents or information is not provided - requests for them)
  3. Institution issuing permit-hygiene passport informs the applicant about inspection of the conditions of services provided for by the applicant, the date and time of inspection
  4. Institution issuing permit-hygiene passport conducts inspection of the conditions of services at the place of the business (if violations of regulations (hygiene norms and other legislation) is found during the inspection, a period of 45 days is granted to resolve violations)
  5. Institution issuing permit-hygiene passport informs the applicant about the decision (a refusal to issue a permit-hygiene passport is decided if recipient of service did not resolved violations of regulation during the granted period of 45 days)
  6. Recipient of service pays state fee (if decision was to issue permit-hygiene passport)
  7. Permit-hygiene passport is given to recipient of service

8.

List of documents to be submitted to obtain a permit

  1. An application for a permit-hygiene passport
  2. For natural persons - a copy of identity document
  3. A document (or its copy) proving the right to use the premises in which the applicant intends to provide services, unless the applicant owns the premises
  4. Cadastral measurement file (or its copy) of the premises in which the applicant intends to carry on the business. Cadastral measurement (inventory) file consists of explanations and blueprints (drawings of the premises) and is made by surveyor.   Requirement of cadastral measurement file does not apply to simple structures.

Applicant has the right to submit a complaint to the Administrative Disputes Commission in accordance with the procedure established by the Law on Administrative Dispute Commissions of the Republic of Lithuania and to the Administrative Court in accordance with the Law on Administrative Proceedings of the Republic of Lithuania. Applicant has the right to file a complaint to the Seimas Ombudsman by fallowing procedure established by the Law on the Seimas Ombudsmen on abuse of public servants, bureaucracy or other violations of human rights and freedoms in public administration

A tax, fixed by state, must be paid before corrected permit-hygiene passport or duplicate of  permit-hygiene passport can be withdrawn. Description of process for obtaining a service

Correction of permit-hygiene passport:

  1. Recipient of service submits a request and documentation that verifies  information that was changed or was miswriten
  2. Provider of service inspects the submitted documents (if documents or information that is required was not provided - requests for them )
  3. Provider of service informs recipient of service about the decision
  4. Recipient of service pays state fee (recipient of service does not pay the state fee if the mistake was made by provider of service)
  5. Corrected permit-hygiene passport is issued by issuing a new permit-hygiene passport
  6. Previous permit-hygiene passport is revoked
  7. Corrected permit-hygiene passport is given to recipient of service

 

Issuance of duplicate of permit-hygiene passport:

  1. Recipient of service submits a request for issuance of duplicate of permit-hygiene passport
  2. Provider of service reviews submitted documents
  3. Provider of service informs recipient of service about the decision
  4. Recipient of service pays state fee
  5. Duplicate of permit-hygiene passport is given to recipient of service

 

Suspension of permit-hygiene passport:

  1. Recipient of service submits a request for suspension of permit-hygiene passport
  2. Provider of service reviews submitted request  
  3. Provider of service suspends  permit-hygiene passport
  4. Provider of service informs recipient of service about the suspension of permit-hygiene passport

 

Revocation of suspension of permit-hygiene passport:

  1. Recipient of service submits a request for revocation of suspension of permit-hygiene passport, declaring that the conditions of bussines has not changed prior to the suspension of permit-hygiene passport
  2. Provider of service reviews request and declaration
  3. Provider of service informs recipient of service about the decision
  4. Suspension of permit-hygiene passport is revoked (if decision was to revoke the suspension)

 

Revocation of permit-hygiene passport:

  1. Recipient of service submits a request for revocation of permit-hygiene passport
  2. Provider of service reviews request
  3. Provider of service informs recipient of service about the decision of revocation of permit-hygiene passport
  4. Permit-hygiene passport is revoked

  1. Recipient of service submits a request and environmental impact assessment report of the proposed economic activity with evaluated proposals from concerned citizens
  2. Provider of service reviews submitted documents
  3. Conclusion is provided to recipient of service (or a reasoned demand to correct or supplement report, the recipient then resubmits the corrected or supplemented report to the provider of service, which reviews resubmitted report within 10 work days and provides the conclusion to the recipient of service)

Beneficiary of taxes, fees and contributions is State Tax Inspectorate under the Ministry of Finance of the Republic of Lithuania, institutions identification code 188659752. In the payment order, when paying taxes, fees and contributions to the budget collection accounts of the State Tax Inspectorate under the Ministry of Finance of the Republic of Lithuania, you have to indicate:

  • the name of the beneficiary
  • account number of budget collection account
  • payer’s identification code
  • payers name
  • payments code
  • amount

If taxes, fees and contributions are paid for other persons (businesses or citizens), the extended payment order form, including the identification code, name and address of the originator of the payment shall be indicated

Description of process for obtaining a service

1. Recipient of service submits request to issue a certificate (or to extend it)

2. Provider of service inspects the submitted information

3. Provider of service agrees with the recipient of service on time and date of inspection of ship

4. Provider of service inspects ship and documents

4.1. ship Sanitation Control Exemption Certificate is issued when the ship is without cargo or there is only ballast or other material, which do not preclude to conduct detail inspection and there are no regulation violations which may pose a risk to public health

4.2. ship Sanitation Control Certificate is issued when violations of regulation that may pose a risk to public health are found during inspection. Imposed, implemented control measures and dates of reinspection is noted on Ship Sanitation Control Certificate.  If there are no possibilities for a ship to implement the control measures, the violations of regulation are indicated in the annex of Ship Sanitation Control Certificate.

4.3. term of certificate may be extended one time for one additional month, if inspection of the ship can’t be conducted or if there are no possibilities to implement the control measures

5. Provider of service informs recipient of service about the decision

6. Recipient of service pays state fee (if decision was to issue certificate)

7. Certificate is given to recipient of service

8.

1. Request for a consultation (general request form), which contains:

1.1 text written in Lithuanian language or have a translation into Lithuanian language, which authenticity must be certified in the prescribed order by the Law on the Notary of the Republic of Lithuania

    1. text is readable
1.3. recipient of service: name, surname, place of residence (if recipient of service is citizen) or company name, company’s registration code, address (if recipient of service is business), contact details and signature of recipient of service.
2. Electronically written request must be signed by a qualified electronic signature or formed by electronic means, which makes it possible to ensure the integrity and irreplaceability of the text
3. Request sent by e-mail must be sent to the official e-mail address, specified on the home page of website of the National Public Health Center under the Ministry of Health 
4. When the representative of a person applies to  National Public Health Center under the Ministry of Health  on behalf of the person represented, there must be indicated name, surname, place of residence (by which a person wants to get an answer) as well as the name and surname of represented person (if applied on behalf of individual) or name (if applied on behalf of legal entity). Representation confirming document (or the copy) must be submitted.
5. Written request can also be submitted by the recipient of service (or its representative) at the reception of National Public Health Center under the Ministry of Health
